NEED HELP~ Authorization Form for paypal?

Hello there!, When I tried to download the Authorization Form.
As you can see the picture below this always pops up when I always try to download it
I sent a message to the hs5 and told them that it won't download so I just filled out the name,which credit card is using,location,etc...,
and told them that I sent a message instead of downloading the Authorization Form. BUT when I just check the tiny box * MINE* (means the credit card's mine*)
and I browse it and chose the driver license picture. And then submit after 24 hrs, and of course it's failed. I need help please?.. I'm using my sister's credit card
so I had to check the tiny box * NOT MINE* but Authorization Form won't download. Need help please?

i would like to ask u to use your internet explorer and dont use the mozilla
please tell me, if u are success or not after using the internet explorer

also regarding to the verification, u need to scan your driver license or your identity card
also you need to scan the front of your credit card (just the fron of your credit card)
do not ever scan the back of your credit card, since on there u can see the cvv
and, cmiiw, as i know u need to show them, that ur credit card and your identity card have a same name

so please use the internet explorer

I used internet explorer many times and it does the same thing :'( :'(  i am using my
sister's credit card..she used it also for her one time verification and it did succeed but.. when i tried mine!
the authorization form doesn't even do anything!!...it doesnt download!!...and when i tried to not to download it
and put hte driver's license picture..it just keeps on failing!..i sent a message to the G.Ms..and i told them about it so I
include the information also but they did not respond when I checked my account the other day..it just keeps on failing i dont know what
to do now =/

as i remember, u need to put the right phone number on ur information
they will call u to make sure that the phone is your's
and they will do a confirmation about ur verification

ok now i will help u to download the form
i think u dont have the adobe reader/pro
cos i saw ur  mozilla failed to open the *.pdf file
please kindly install adobe pro (if u want to writing the downloaded file)
or u can find a free software for pdf writter,editor and reader

when u click the download form, the browser automaticly try to open the pdf file on ur browser
now u can download it without open it from ur browser
first u click the right click on ur mouse or ur touch pad, and the choose the save link as..
save it in ur folder, and install the software for pdf editor

any question pls let me know
